면역혼탁측정법을 이용한 HemosIL Free Protein S 시약의 수행능 평가

Abstract

Purpose:

It is necessary to measure free protein S antigen levels in plasma for diagnosing protein S deficiency. The aim of this study was to evaluate the performance of HemosIL Free Protein S reagent (Instrumentation Laboratory, Lexington, MA, USA) which is an automated latex ligand immunoassay for the quantitative determination of free protein S.

Methods:

HemosIL Free Protein S was evaluated for its precision, linearity, and reference range and compared to the HemosIL ProS, which is a reagent for the measurement of protein S activity in plasma, following the Clinical and Laboratory Standards Institute guidelines. The HemosIL Free Protein S was also compared to the STA-Liatest Free Protein S (Diagnostica Stago, Taverny, France), which is another reagent used in testing free protein S antigen levels.

Results:

The within-run, between-run, and total coefficients of variation (CV) for HemosIL Free Protein S assay were 5.3%, 5.9%, and 7.9% at the low level, respectively, and 3.3%, 2.4%, and 4.1% at the high level, respectively. The linearity was in the range of 10.9-126.1% with a high correlation coefficient (R) of 0.997. The comparison between HemosIL Free protein S and ProS showed good correlation (R= 0.866). An excellent correlation (R= 0.931) was found between HemosIL Free protein S and STA-Liatest Free Protein S. The reference ranges for women and men were 50.8-116.9% and 69.4-138.3%, respectively.

Conclusion:

HemosIL Free Protein S for measuring free protein S antigen in plasma had excellent precision and linearity. HemosIL Free Protein S was considered useful in detecting protein S deficiency.

서론

단백 S는 635개의 아미노산으로 이루어진 75 kDa 크기의 비타민 K 의존 혈장 당 단백으로 간에서 주로 합성되고 단백 C의 인지질에 대한 친화력을 증강시켜 활성화된 5번, 8번 응고인자를 불활성화시킴으로써 혈액응고를 억제하는 보조인자로 작용한다[1]. 혈중 단백 S의 약 60%는 C4b-결합단백(C4b-binding protein, C4b-BP)과 결합하여 존재하고 나머지 40% 정도만 유리형으로 존재하는데 단백 C의 보조인자로서의 역할은 유리형이 가지고 있다[2]. 여러 병리적, 생리적 상태에 따라 결합형과 유리형의 비율이 변함으로써 단백 C 억제 경로(protein C inhibitory pathway)를 효율적으로 조절하게 된다[3].
단백 S의 결핍은 항원과 활성이 모두 감소된 제1형, 전체 항원량과 유리형의 항원량은 정상이나 활성이 감소된 제2형, 전체 항원량은 정상이나 유 리형의 항원량이 감소되어 활성이 감소되는 제3형으로 나뉜다. 이 중 제1형이 2/3를 차지하고 나머지 1/3은 제3형이며 제2형은 드물다[4,5]. 단백 S 결핍은 혈전 호발군에서 7-12% 정도의 유병률을 보인다[2].
단백 S 결핍을 진단하기 위해서는 응고활성도와 항원량을 검사하는데 단백 S 활성도 검사는 거짓으로 낮은 값을 나타내기도 하여 단백 S 결핍을 진단하기 위해서는 유리형 항원을 먼저 측정하도록 권장하고[4,6] 이것이 유의하게 감소된 경우 추가로 단백 S 활성도와 총단백 S의 항원량을 측정하도록 한다[2].
이 연구에서는 유리단백 S의 항원량을 측정하는 HemosIL Free Protein S (Instrumentation Laboratory, Lexington, MA, USA) 시약의 수행능을 평가하고자 하였다.

방법

기기 및 시약

유리단백 S의 항원량을 측정하기 위해서 HemosIL Free Protein S (Instrumentation Laboratory) 시약과 ACL-TOP 500 CTS (Instrumentation Laboratory) 자동혈액응고분석기를 사용하였다. 또한 동일한 장비에서 유리단백 S의 활성도 측정을 위해 HemosIL ProS (Instrumentation Laboratory) 시약을 사용하였다. 동일한 방법으로 유리단백 S의 항원량을 측정하는 다른 시약과의 비교평가를 위해서 STA-R (Diagnostica Stago, Taverny, France) 자동혈액응고분석기와 STA-Liatest Free Protein S (Diagnostica Stago) 시약을 사용하였다.
HemosIL Free Protein S는 혈장 내의 유리단백 S 항원을 정량하기 위해 라텍스 면역측정법을 이용한다. 칼슘이온의 존재하에 라텍스에 흡착되어 있는 C4b-BP이 환자의 혈장에 있는 유리단백 S와 강하게 결합하고, 이어서 단백 S에 대한 단클론항체로 코팅되어 있는 두 번째 라텍스 시약과 반응하여 응집을 촉진시키게 된다. 응집의 정도는 검체 내에 존재하는 유리단백 S의 양에 비례한다[5,7].
HemosIL ProS 시약은 단백 S의 활성도를 측정하기 위해서 조직인자, 인지질, 칼슘이온과 단백 C가 존재하는 상태에서 프로트롬빈시간(prothrombin time, PT)이 지연되는 정도를 측정한다. 단백 S의 활성도는 단백 S 결핍 혈장에 환자의 검체를 첨가하여 측정되는 응고시간의 지연에 비례한다[4,8,9].

검체

평가에 사용된 환자 검체는 Clinical and laboratory standards institute (CLSI) 지침[10]에 따라 3.2% 구연산나트륨이 든 진공채혈관(Becton Dickinson Ltd, Franklin lakes, NJ, USA)에 채혈하였다. 혈소판결핍혈장을 얻기 위해 원심분리(10분, 2,500 g)하였으며 검사할 때까지는 실온에서 보관하였고 4시간 이내에 검사를 시행하였다. 비교평가를 위해서 -70°C에 검체를 보관하였고 검사 시에는 37°C에서 해동하고 잘 섞은 후 2시간 이내에 검사를 시행하였다.

정밀도

CLSI 지침[11]에 따라 수행하였으며 제조사에서 제공하는 25-45% 농도범위의 Low Abnormal Control Assayed (Instrumentation Laboratory)와 80-120% 농도범위의 Normal Control Assayed (Instrumentation Laboratory) 두 가지 정도관리물질을 이용하였다. 정밀도 평가를 위해 동일한 검체를 하루 2회, 1회 3번씩 6일간 측정하여 평균, 표준편차, 변이계수 등을 산출하였다.

직선성

CLSI 지침[12]에 따라 수행하였으며 측정값이 높은 환자검체를 희석하여 제조사에서 제공한 측정가능범위를 평가하였다. 측정값이 126.1%인 검체를 100.9% (0.8 희석), 75.7% (0.6 희석), 50.4% (0.4 희석), 25.2% (0.2 희석), 12.6% (0.1 희석), 6.3% (0.05 희석) 농도로 단계 희석하여 평가하였으며 최적의 다항식이 일차선형인 경우 해당범위에서 직선성이 유지되는 것으로 판정하였다.

참고범위

CLSI 지침[13]을 참고하여 참고범위를 설정하였다. 건강한 20-50대의 남성과 여성 각각 120명의 검체를 사용하여 유리단백 S 항원량을 측정하고 평균, 표준편차, 최소값, 최대값을 구하였다.

비교평가

단백 S 검사가 의뢰된 20명의 환자 검체에서 ProS 시약과 Free Protein S 시약을 사용하여 각각 단백 S 활성도와 항원량을 측정하여 그 결과를 비교하였다[14]. 또한 동일한 방법으로 단백 S의 항원량을 측정하는 다른 제조사의 시약과 그 측정 결과를 비교하기 위해 40명의 환자 검체를 이용하여 단백 S 항원량을 측정 후 평균, 표준편차, 상관계수 등을 구하였다.

통계

통계처리를 위해 Microsoft Office Excel 2007 (Microsoft Corporation, Redmond, WA, USA), MedCalc (MedCalc Software, Ostend, Belgium), SPSS v18.0 (SPSS Inc., Chicago, IL, USA) 등의 프로그램을 이용하였다. 참고치 범위설정을 위해 Student’s t-test, 나이와 유리단백 S 항원농도의 상관성을 보기 위해 Pearson correlation test, 단백 S 활성도와 항원량 그리고 서로 다른 제조사의 단백 S 항원량 검사를 비교하기 위해서 Wilcoxon signed ranks test 통계기법을 사용하였다. 검사의 유의수준(P value)이 0.05 미만일 때 통계학적으로 유의하다고 판단하였다.

결과

정밀도

검사차례내(within-run) 변이계수는 저농도 5.3%, 고농도 3.3%이었고 검사수행내(between-run) 변이계수는 저농도 5.9%, 고농도 2.4%, 총변이계수는 저농도 7.9%, 고농도 4.1%이었다(Table 1).

직선성

희석한 7개 검체의 측정값은 10.9-126.1%로 최적의 다항식이 일차선형이었고 회귀방정식의 상관계수가 0.997으로 직선성을 유지하였다(Fig. 1).

참고범위

건강한 성인 남녀 각각 120명을 대상으로 시행한 결과 두 군 모두 정규분포를 보이며 여성은 50.8-116.9%, 남성은 69.4-138.3%로 참고범위를 산출하였다(P< 0.05, Table 2).

단백 S 활성도 측정시약과 유리형 항원 측정시약의 비교

HemosIL ProS 및 HemosIL Free Protein S 시약을 이용하여 20명의 환자 검체를 검사한 결과 각각 평균 62.2%, 71.8%, 최소값 16.6%, 16.7%, 최대값 111.5%, 125.7%를 보였다. 두 시약에 의한 측정값 사이에는 0.866의 상관계수를 보이고 Pro S 시약이 평균적으로 16.4% 낮은 수치를 보였다(P=0.005, Fig. 2).

타제조사 단백 S 유리형 항원 측정시약과의 비교

ACL-TOP 500 CTS 장비에서 HemosIL Free Protein S 시약으로 측정한 값은 평균 84.6%, 최소값 26.0%, 최대값 130.0%이었으며 STA-R 장비에서 STA-Liatest Free Protein S 시약으로 측정한 값은 평균 75.5%, 최소값 28.8%, 최대값 121.9%이었다. 두 시약으로 측정한 값 사이에는 0.931의 상관계수를 보였으며 HemosIL Free Protein S시약으로 측정한 값이 평균 9.5% 높게 측정이 되었다(P< 0.001, Fig. 3).

고찰

단백 S의 활성도를 측정하는 기능적 방법은 활성화 단백 C (activated protein C, APC)의 보조인자로서의 활성을 측정한다[15]. 이 검사방법은 활성화 부분트롬보플라스틴시간(activated partial thromboplastin time, APTT) 또는 PT와 같은 응고시간을 기반으로 하기 때문에 Factor V Leiden, 8번 응고인자의 증가, 루푸스항응고인자, 응고인자결핍, 트롬빈억제제(direct thrombin inhibitor) 등 여러가지 원인에 의해 검사결과가 많은 영향을 받는다[16]. 전자의 두 요인에 의해서는 단백 S 활성도가 거짓으로 감소하고 나머지 후자의 요소들에 의해서는 거짓으로 증가할 수 있다[4,16-18]. 따라서 단백 S 측정은 간섭요인에 의한 영향을 받지 않는 면역학적 방법에 의한 항원량의 측정이 필요하다.
유리단백 S 항원량이 의미있게 낮은 경우 정맥혈전의 위험률이 증가한다[5]. 제1형과 제3형 단백 S 결핍은 모두 유리단백 S 항원량의 감소를 보이는데 특히 제3형 단백 S 결핍은 총단백 S 항원량이 정상이므로 총 단백 S 보다는 유리단백 S 항원량을 측정할 필요가 있다[19]. 따라서 단백 S 결핍을 진단하기 위해서 유리단백 S 항원이 총단백 S 항원보다 더 나은 척도라고 할 수 있다[5]. 저자들은 유리단백 S 항원량을 측정하는 HemosIL Free Protein S 시약의 검사 수행능을 평가하였다.
HemosIL Free Protein S 시약으로 두 가지 농도의 물질에서 시행한 검사결과를 통해 검사차례내 변이계수, 검사수행내 변이계수, 총 변이계수는 그 값이 각각 10% 이하로 양호한 정밀도와 우수한 직선성을 보여주었다. HemosIL Free Protein S 시약을 이용한 검사는 여러가지 요인에 의해 영향을 받는 C4b-BP-단백 S 복합체의 해리과정과 검사 전에 검체를 희석하는 단계가 없으며 두 라텍스 시약이 반응하는 시간도 각각 2분 이내로 비교적 짧기 때문에 재현성이 높고 시간적인 면에서 효율적인 검사라고 할 수 있다[5,20,21].
일반적으로 유리단백 S 항원 농도는 여성이 남성보다 더 낮은 것으로 알려져 있고[22,23] 본 연구에서도 여성의 유리단백 S 농도가 남성보다 더 낮게 측정되었다(P< 0.001). 남성과 달리 여성에서는 연령, 경구피임제 복용, 폐경, 호르몬 치료유무에 따라 단백 S항원의 농도차이를 보일수 있는데[17,22,23] 본 연구에서는 기존 보고와 같은 연령에 따른 차이를 확인할 수 없었다(P= 0.942). 이는 각 연령별 검체수가 적었음와 관련이 있을 것으로 보이며 또한 참고범위 평가를 위해 참여한 정상인들의 구체적인 임상자료를 얻지 못했기 때문에 그것들이 단백 S 항원의 농도에 미치는 영향을 알 수 없었기 때문으로 생각된다.
이 연구에서 단백 S의 활성도는 유리단백 S 항원량에 비해서 16.4% 낮은 값을 보였는데(P= 0.005) 이는 앞에서 언급한 바와 같이 여러 가지 요인이 단백 S 활성도에 영향을 미친 것으로 생각된다. 특히, 냉동보관한 검체를 다시 녹인 후 검사를 하면 단백 S의 활성도가 현저히 감소함을 알 수 있었다. 유리 단백 S의 항원량을 측정하는 HemosIL Free protein S 시약과 STA-Liatest Free Protein S 시약을 비교했을 때 HemosIL 시약이 평균 9.5% 높은 값을 보였다(P< 0.001). 최근의 한 연구도[24] 비슷한 결과를 보였는데 모든 상품화된 시약들이 국제적인 기준을 따라 보정하여 제조되더라도 시약과 장비의 차이에 따라 상이한 결과를 보이는 것으로 생각된다[25].
결론적으로 유리단백 S 항원을 측정하는 HemosIL Free Protein S 시약은 우수한 정밀도와 직선성을 보여 주었고 단백 S 결핍증을 진단하는데 도움이 되리라 생각된다.
